PT-141 and Perimenopause: More Than a Libido Boost (My Honest Experience)

Why I Started PT-141

Perimenopause snuck up on me. My libido dropped, my mood dipped, and I felt a little disconnected from the people I love. When I brought this up with my doctor, PT-141 (bremelanotide) came up as an option, and my doctor prescribed it for me.

I don't take it every day. I use it here and there, and that has worked well for me.

What PT-141 Is (and Its FDA History)

PT-141 is a synthetic peptide that works in the brain, not on blood flow the way Viagra does. It activates melanocortin receptors (mainly MC4R), which play a role in sexual desire and arousal.

Here's a quick look at its history:

  • It was developed by Palatin Technologies from an earlier compound, Melanotan II, which was originally being studied for tanning.

  • Early human studies in the 2000s showed it increased sexual arousal in both men and women.

  • It was first tested as a nasal spray, but development switched to an injection after concerns about blood pressure.

  • In the RECONNECT trials, about 1,200 premenopausal women with low sexual desire took bremelanotide or a placebo for 24 weeks. The women on bremelanotide reported more desire and less distress about their low desire.

  • In June 2019, the FDA approved it under the brand name Vyleesi for premenopausal women with acquired, generalized hypoactive sexual desire disorder (HSDD). It's an injection you take as needed, at least 45 minutes before sex.

The FDA approval covers premenopausal women. My doctor prescribed it for my situation in perimenopause, and that's a conversation worth having with your own provider.

It's Not Just About Libido for Me

Yes, it helps my libido. What surprised me most was everything else.

On days I use it, my mood feels lifted. I find myself wanting to engage more with my kids. I play more, talk more, and I'm more present with them. I feel more connected and more empathetic toward my friends and family, too.

For me, that sense of closeness with the people I love feels like a natural antidepressant. Connection and bonding are tied to oxytocin, often called the "bonding hormone," and some animal research suggests the melanocortin system PT-141 acts on may interact with oxytocin pathways in the brain. PT-141 hasn't been studied as a mood treatment in people, so I can only tell you what I've felt: warmer, more open, and more present.

Perimenopause Support

Perimenopause can affect your mood, energy, intimacy, and how connected you feel in your relationships. PT-141 has helped me with several of these, especially desire, mood, and feeling like myself around the people I love. Every woman's perimenopause is different, so your experience may not look like mine.

Sexual Health Is Longevity Health

Sexual health isn't a luxury. It's part of longevity. The happier you are in your connection with your spouse or partner, the happier and healthier your life tends to be, and the research backs this up.

The Harvard Study of Adult Development, one of the longest-running studies of adult life (started in 1938), found that people's satisfaction with their relationships at age 50 predicted their physical health at 80 better than their cholesterol levels did. A 2024 study in the Journal of Psychosexual Health looked at more than 14,000 U.S. adults and found that women who had sex less than once a week had about a 70% higher risk of death over the study period than women who were more sexually active.

These studies show a strong link, not a guarantee, but the message is clear to me: intimacy and connection are part of how we age well.

The Side Effects I've Experienced

I'll be honest, it's not side-effect free.

  • Nausea: I do get nauseous when I take it. That lines up with the research. Nausea was the most common side effect in the clinical trials, reported by about 40% of women. It was usually mild to moderate and most common with the first dose.

  • Flushing: I get a little flushed. About 1 in 5 women in the trials reported flushing.

For me, those are the worst side effects I've had.

Other side effects reported in the research include headache, injection-site reactions, and a temporary rise in blood pressure. With frequent use, some women also got darker patches of skin. It's not recommended if you have uncontrolled high blood pressure or heart disease, which is one more reason to go through your doctor.

Final Thoughts

I started PT-141 to support my libido in perimenopause, but it has become something more for me. It gives me a little lift in mood, more patience and warmth with my kids, and more connection with the people I love. I'm not a doctor, just a woman sharing what has worked for me. If you're curious, talk to your provider about whether it's a fit for you. And if you've tried it, I'd love to hear about your experience reina@reipilates.com
