Why Vitamin B12 Deserves a Spot on Your Longevity Checklist


If there’s one nutrient that quietly becomes more important with every passing decade, it’s vitamin B12. It doesn’t get the same buzz as NAD+ precursors or peptides, but low B12 is one of the most common — and most fixable — contributors to fatigue, cognitive decline, and nerve issues in people over 50.

Why B12 Needs Change With Age

B12 absorption depends on stomach acid and a protein called intrinsic factor, both of which naturally decline as we age. On top of that:

• Medications matter. Long-term use of proton pump inhibitors (for acid reflux), metformin (for blood sugar), and some other common prescriptions all interfere with B12 absorption.

• Diet shifts. B12 is found almost exclusively in animal products, so anyone eating less meat, dairy, or eggs — by choice or appetite changes — is at higher risk.

• Gut changes. Conditions like atrophic gastritis, which become more common with age, reduce the stomach’s ability to release B12 from food.

The result: an estimated 10–30% of adults over 50 have some degree of B12 deficiency, and many don’t know it because symptoms creep in slowly.

The Benefits — For Men and Women Alike

B12 plays a role in several systems that matter enormously for healthy aging, and the benefits apply equally regardless of sex:

Brain and cognition. B12 is essential for maintaining the myelin sheath that insulates nerve cells and for regulating homocysteine, an amino acid linked to cognitive decline and dementia risk when elevated. Adequate B12 supports memory, focus, and mood.

Energy and red blood cells. B12 is required to make healthy red blood cells. Deficiency causes megaloblastic anemia, which shows up as persistent fatigue, weakness, and shortness of breath — often mistaken for “just getting older.”

Nerve health. B12 protects the peripheral nervous system. Deficiency can cause numbness, tingling, or a pins-and-needles sensation in the hands and feet, and left untreated, nerve damage can become permanent.

Cardiovascular support. By helping regulate homocysteine, B12 also plays a supporting role in cardiovascular health — relevant for both men and women, whose cardiovascular risk profiles shift after 50.



Bone health. Some research links low B12 to reduced bone mineral density, which is a particular concern for postmenopausal women already facing elevated osteoporosis risk.

Side Effects and Safety Considerations

The good news: B12 is water-soluble, and your body excretes what it doesn’t need, making toxicity extremely rare. That said, it’s not entirely risk-free:

• Injection site reactions (redness, soreness) are the most common issue with B12 shots.

• Skin issues — mild acne or rosacea flares have been reported in some people on high-dose B12.

• Interactions — B12 can interact with certain medications, including some antibiotics, metformin, and drugs that reduce stomach acid, either affecting absorption or requiring dose adjustments.

• Masking other deficiencies — high-dose B12 supplementation can normalize blood counts while masking an underlying folate deficiency, which is why testing matters more than guessing.

For most people, standard doses are very well tolerated. Problems are far more often linked to deficiency than to supplementation.

The Bloodwork You Actually Need

This is the part that gets skipped too often: don’t supplement blind. A proper picture requires more than a basic B12 level, since serum B12 alone can miss functional deficiency. Ask a qualified physician about:

• Serum B12 — the baseline screen

• Methylmalonic acid (MMA) — rises when B12 is functionally low, even if serum levels look normal

• Homocysteine — elevated levels can signal a B12 or folate problem and carry independent cardiovascular and cognitive risk

• Complete blood count (CBC) — checks for the anemia pattern associated with deficiency

• Folate levels — tested alongside B12, since the two deficiencies mimic and mask each other

A physician can also help identify why levels are low — absorption issue, diet, medication, or an underlying condition — and choose the right form and dose (oral, sublingual, or injection) based on your specific case.
